Matthew Kershaw reflects on provider readiness seminars ...

Matthew Kershaw, Director of Provider Delivery, has taken time out to share his reflections on a series of seminars which the Department has been running in partnership with NHS Confederation across the country to discuss provider readiness for the new landscape.

I attended four out of five of the seminars for senior leaders of foundation trusts and NHS trusts to discuss their readiness to tackle the challenges of the new provider landscape.

We had interesting and stimulating debates about the current health economy and its future, looking at internal and external issues that are concerning providers.

One of the big challenges facing NHS trusts is developing a five-year business plan, which is clinically and financially robust, when so much in the system is changing.? Emerging clinical commissioning groups across the country are also at different levels of maturity and development, so NHS trusts ?will need to factor this in when discussing longer-term strategies.

Commissioners will dedicate time and resources to building relationships with their local providers, so that the new approach to delivering care will really place patients at the heart of the decisions.? This will allow organisations to look beyond organisational boundaries and take account of all the options available across the system.? We will not achieve integrated services or long term clinical and financial sustainability without it.

From an internal perspective, leaders were concerned about the role of governors, how to attract people who have a wider spectrum of backgrounds.? It is the responsibility of individual organisations to build strong and effective relationships with their council of governors.? It?s a case of you get out what you put in ? Governors are a connection with a trust?s local population and by building better relationships with them can support the development and implementation of your strategy.

With the Health and Social Care Bill having successfully passed through Parliament, the health landscape will begin to operate very differently.? Therefore, all parts of the health system need to look beyond their own boundaries. They must all work together to deliver care for patients which is focussed on the best possible outcomes irrespective of organisational boundaries.

Army sergeant to be charged in Afghan killings

WASHINGTON (AP) ? Army Staff Sgt. Robert Bales is expected to be told he faces 17 counts of murder and six counts of attempted murder, along with other charges, in connection with a shooting rampage in two southern Afghanistan villages that shocked Americans back home and further roiled U.S.-Afghan relations.

The charges come almost two weeks after the massacre in which Bales allegedly left his base in the early morning hours and shot Afghans, including women and nine children, while they slept in their beds, then burned some of the bodies.

Bales will be read the charges on Friday at the military prison at Fort Leavenworth, Kan., where he has been held since being flown from Afghanistan last week, a U.S. official said.

In addition to murder and attempted murder, the charges will include six counts of aggravated assault as well as a number of other violations of military law, said the official, who spoke on condition of anonymity to discuss the charges before they were announced.

The 38-year-old soldier and father of two, who lives in Lake Tapps, Wash., faces trial under the Uniform Code of Military Justice, but it could be months before any public hearing.

Military authorities had originally said Bales was suspected in the killing of 16 Afghan villagers, nine children and seven adults. They changed that Thursday to 17, raising the number of adults by one but without explaining how the change came about. It's possible some of the dead were buried before U.S. military officials arrived at the scene of the carnage. Six Afghans were wounded in the attack.

Bales' civilian attorney, John Henry Browne, said he wouldn't comment on the charges because he has not been officially provided a copy of them. He said he spoke Thursday with prosecutors, who told him they will formally present the charges Friday.

The slaughter of Afghan villagers was yet another blow to U.S-Afghan relations, following a series of missteps, including the mistaken burning of Qurans, which prompted violent protests and revenge killings of American troops in the war zone.

The killings also prompted renewed debate in the United States about health care for the troops, who have experienced record suicide rates and high rates of post-traumatic stress and brain injuries during repeated deployments over a decade of the wars in Iraq and Afghanistan.

Bales was on his fourth tour of duty in a war zone, having served three tours in Iraq, where he suffered a head injury and a foot injury. He was assigned to the 2nd Battalion, 3rd Infantry Regiment of the 3rd Stryker Brigade Combat Team, of the 2nd Infantry Division, which is based at Joint Base Lewis-McChord in Washington.

Browne has portrayed his client as a patriot, loving father and devoted husband who had been traumatized by a comrade's injury and sent into combat one too many times.

But there have been conflicting reports about what exactly Bales saw relating to the comrade's injury. A U.S. defense official said that while it is likely that a soldier from Bales' unit, based in the Panjwai district of Kandahar province in southern Afghanistan, suffered a leg wound a day or two before the March 11 shootings, there is no evidence that Bales witnessed it or the aftermath, or that it played any role in his alleged actions.

The official spoke on condition of anonymity to discuss an internal review.

Afghan officials have asked the United States for some role in the criminal proceedings, perhaps as observers, and to be kept up to date on the case. The government of Afghan President Hamid Karzai has not demanded that Bales be turned over to the Afghan justice system, although some in the country's parliament did. The Afghans have also urged a fast resolution of the case.

Browne has also said that Bales has some memories from before the killings and some from after but very little of the time when the military says he went on the shooting rampage.

"I'm not putting the war on trial," Browne has said, "but the war is on trial." He added: "If I can help create a discussion about the war, that would be a great way for me to go out."

Army officials have said Bales was cleared for return to duty after the head injury he suffered in Iraq.

Bales joined the Army in 2001 after a Florida investment business failed and after he had worked with a string of securities operations. Bales and a broker at one company were hit in 2003 with a $1.5 million arbitration ruling after an elderly couple charged that their holdings were decimated.

He also was arrested in 2002 for the drunken assault of a casino security guard and had to complete an anger management class. There also are reports of a second incident involving alcohol, although Bales was never formally charged.

A sheriff's department report released Thursday says Robert Bales was accused in 2008 in Washington state of shaking hands with a woman, pulling her hand into his crotch and then punching and kicking her boyfriend. It described Bales as "extremely intoxicated."

(AP) ? You can't hold the London Olympics without the British royals, now can you?

The two have been intertwined over the years like the Olympic rings themselves. At the London Games this summer, they'll be everywhere ? Wills and Kate, Harry and Zara, the queen and Prince Philip ? posing, pronouncing, maybe even participating.

These games will showcase a whole new royal generation: the elegant Duchess of Cambridge, whose fairy-tale wedding last year was watched by hundreds of millions; the irrepressible Prince Harry, at 27 one of the world's most eligible bachelors; and another one of Queen Elizabeth II's grandchildren, Zara Phillips, who is seeking a berth on the British equestrian team just like both her parents did years ago.

Prince William, his wife Kate and Harry will all be Olympic ambassadors. It's not clear what any of the royals will actually be doing or where they will be appearing, but British officials and tourism experts consider their presence vital.

"They're great for attracting publicity," said Joe Little, the managing editor of Majesty magazine. "Everybody wants to see the Duchess of Cambridge. ... It's the youth-and-glamour thing."

Visit Britain says some 30 million people come to the country each year to see its cultural heritage ? such as Buckingham Palace, the changing of the guard and the Beefeaters at the Tower of London.

Britain's culture and heritage sites bring in 4.5 billion pounds ($7 billion) of the 17 billion pounds ($26.6 billion) spent by overseas visitors annually. This being an Olympic year ? and the queen's 60th Jubilee anniversary to boot ? the focus on U.K. palaces and royals will be magnified.

To make the most of things, the monarch plans to open Buckingham Palace to accommodate Olympic activities.

A tourism promotion is being launched to coincide with the July 27-Aug. 12 games: the GREAT campaign, as in Great Britain. It hopes to capitalize on the Olympic excitement and bring an extra 2 billion pounds ($1.5 billion) of tourism spending into Britain's beleaguered economy.

Harry did his bit to build excitement last week in Jamaica, donning a jersey emblazoned with Jamaica's green, black and gold colors and clowning around with 100-meter champion Usain Bolt. They joked and "raced" one another, meeting on the track to strike a mock lightning bolt pose, Bolt's signature.

"I am not directly involved in the (Olympic) organization, otherwise who knows what might happen?" the helicopter pilot-trainee prince joked. "If work permits me, I will definitely get the chance, hopefully, to visit as many events as possible."

The royals and the Olympics have long been intertwined, going back to a time when only the very rich could afford to train and remain amateurs.

London took on the Olympics for the first time in 1908, after Mount Vesuvius erupted in Rome, forcing Italy to pull out. King Edward VII lobbied for Britain to host those games, playing a key role in persuading the government to accept the task at a time when the Olympics were largely unknown.

"That kind of gave it legitimacy," said Martin Polley, an Olympic historian at the University of Southampton. The thinking was "if (the royals) are backing it, it must be serious."

London next hosted the games in 1948, with King George VI presiding as the Olympic flag was raised for the first time since the end of World War II.

At the time, the royals were more famous than the games themselves. Fanny Blankers-Koen of the Netherlands, who won four gold medals in 1948 in track and field, told of her disappointment upon hearing the British national anthem after a close race with a British competitor. Blankers-Koen thought she'd lost ? but actually it was just the royal family entering the stadium to take their seats, Polley said.

Queen Elizabeth II will open this summer's games, like her father and great-grandfather once did. She is not new to the job, opening the Montreal Olympics in 1976, where her daughter Princess Anne competed with her horse, Goodwill.

Anne, a member of the 2012 London organizing committee, remembers her Olympic experience fondly, even though she did fall off Goodwill.

"It proved something ... in your sporting experience," she said in a BBC interview. "For the old dinosaurs, the amateurs like me, I think that was a rare treat."

The whole royal family showed up to watch her in Montreal ? a scenario likely to be repeated this summer in London if Anne's 30-year-old daughter Zara makes the team. She had been named to the 2008 Olympic team in Beijing but her horse became injured before the games and could not compete.

Zara's father, Mark Phillips, won equestrian gold in the 1972 Munich Olympics and silver at the 1988 Seoul Games.

She captained her high school field hockey team, and visited Britain's women's team Thursday at Olympic Park to offer a morale boost.

Harry reportedly likes women's beach volleyball ? which just happens to be played in scanty Olympic outfits ? and he'll probably want to see his friend Bolt run.

But the royal impact is more than just being seen. Harry told reporters in Jamaica he wants to encourage children to be active.

"It's massively important to get young kids out doing sport," Harry said. "I know that when I was at school, sport was the best thing. Being stuck in the classroom wasn't."

He paused.

"I probably shouldn't have said that," he grinned.

'Kony' proves social media is youth news source

Word of the "Kony 2012" video spread like lightning via social media networks like Twitter and Facebook, reaching "so many Americans in a relatively short period of time," it demonstrated the "critical role?social media played, especially for adults under age 30," says a new study.

The huge success of the video about Ugandan warlord Joseph Kony "provided striking evidence that young adults and their elders at times have different news agendas and learn about news in different ways," said the?Pew Research Center's Internet & American Life Project in?research released Thursday.

"Those ages 18-29 were much more likely than older adults to have heard a lot about the 'Kony 2012' video and to have learned about it through social media than traditional news sources. Indeed, a special analysis of posts in Twitter showed that it was by far the top story on the platform.?

On Thursday, the 30-minute video, made by the group Invisible Children and released just 10 days ago, was closing in on 80 million?YouTube?views, and had been played more than 17 million times on Vimeo, "making it one of the most viewed videos of all time on those sites," Pew said. ?(According to Visable Measures, "Kony 2012" has had "well over 112 million total views since it was released.")

Younger adults were "also more than twice as likely as older adults to have watched the video itself on YouTube or Vimeo," Pew said.

Pew's report is based on?telephone interviews done March 9 to 11?among a national sample of 814 adults ages 18 years and older living in the continental United States.

In the days following the release of "Kony 2012," Pew said 58 percent of young adults said the heard about the video,?including 40 percent who said they heard a lot about it, compared to?20 percent of those ages 30-40; 18 percent of those ages 50-64; and 19 percent of those ages 65 and older.

"Even more striking is the way people learned about the story," Pew said: 27 percent of young adults "first heard about it through social media such as Facebook or Twitter and another 8 percent learned about it via other Internet sources."

In other words, "The Internet was more than three times more important as a news-learning platform for young adults than traditional media such as television, newspapers, and radio." Only 10 percent of young adults first learned about "Kony 2012" via "traditional media platforms."

Those 50 and older "were much more likely to have learned of the video from traditional sources, especially television: 29 percent of adults ages 50-64 heard from television, newspapers, or radio, compared with 12 percent who heard via the Internet. For adults age 65 and older, 47 percent learned about it from traditional sources and 5 percent learned from Internet sources."

Twitter's role was huge in getting the word out was huge; Pew said there were nearly 5 million tweets about the video the week after it was posted online March 5. The microblogging site was the vehicle used by a young Australian woman to first bring Oprah Winfrey's attention to "Kony 2012," and it boomed from there.

Winfrey tweeted about it to her 9.6 million followers. Also tweeting: Ryan Seacrest, Justin Beiber, Alec Baldwin and Taylor Swift. "The campaign was saluted by White House Press Secretary Jay Carney, who said President Obama offered his congratulations for calling attention to the campaign to stop Kony and his army," reports Pew.

Pew said most of the responses on social media supported the video's message, which is to find and arrest Kony, who is accused of atrocities against Uganda's children:

  • 66 percent of the Twitter conversation from March 5-12 supported the anti-Kony campaign.
  • 17 percent were "skeptical or negative about it."
  • 16 percent were "neutral" (mostly consisting of people asking questions about what was going on).

Some believe Invisible Children's efforts are well-meaning but misguided, with other Ugandan tensions and issues deserving more attention. As more information continues to be reported about issue, the "tone of conversation" may be shifting somewhat, Pew said.

"The first two days after the video was online, when attention on Twitter was relatively modest, 77 percent of the Twitter conversation was supportive compared with only 7 percent that was skeptical or negative. Since March 7, when the response picked up dramatically, the percentage of tweets reflecting skepticism or criticism rose to 17 percent."?

Stuck on what gifts to give for the holidays? How about homemade edible gifts? If you are looking to perhaps save some money, but choose something worthwhile that is sure to be a hit then candy gifts are a great choice. Chocolate bark recipes are easy to make and even more fun to eat. Chocolate bark are broken pieces of chocolate usually mixed with other candies, nuts or other ingredients or flavors. You can package them in decorative gift boxes, cellophane bags or even a combination like cellophane bags inside colorful paper gift bags with Christmas tissue paper, the options are numerous. The options for the chocolate bark recipes themselves is even bigger.

There are three types of chocolate to choose from: dark chocolate, milk chocolate and white chocolate. And even within dark chocolate there are degrees of sweetness, the packaging will tell you the percentage of sweetness. Once you choose the type of chocolate you want you then you choose the other ingredient to mix in it could be anything from prezel pieces, toffee bits, peppermint bits, chocolate chips, raisins, nuts of all kinds, other dried fruits, hard shelled chocolate candies like M&Ms, so on and so on. And there's more than just other ingredients. There's a third element: candy flavorings. You could change the taste of the chocolate by adding a drop or two of oil-based candy flavorings like orange, cherry, strawberry and more. These can be found at your local baking supply store. I also recommend Michaels here in the US. A baking supply store will have more flavors, though and you could always search online.

Not sure which combinations that someone special would like? Then make a few different chocolate bark recipes and mix and match the gifts. Try three different dark chocolate barks like one piece of plain dark chocolate bark, almond dark chocolate bark and cranberry dark chocolate bark. Or perhaps one piece of each chocolate with the same second ingredient or one of each chocolate type with different ingredients in each. For even another idea try the same chocolate type but with three different candy flavorings.

Which chocolate brand works best and in which shape (chocolate chips, wafers, blocks)?

The quality of chocolate is very important. There are some chocolates made for dipping (coating chocolate), there are some made for baking (ex. brownies, mousses, pastries) and there's other chocolate good for eating raw. The best tasting one is the latter which would contain sugar to provide a good flavor. This still throws dark chocolate into the mix which could be bittersweet or semisweet, it would depend on the ratio between chocolate liquor and sugar, the more sugar the sweeter. These chocolate will come in blocks or bars, but if all you have on hand are wafers or chips then they will work just fine. I still would prefer the blocks. As far as the brand for Wafers I would go with Wiltons or Merckens. These are two of the most popular brands on the market. You can buy these at a local baking supply store, Michaels and even Wal-Mart is now carrying Wiltons. For the blocks you could go with Hershey's or Ghiradelli's.

Ready for a recipe? Let's try dark chocolate peanut bark. The crunch of the peanuts and snap of the crisped rice cereal add various textures to enjoy.

Dark Chocolate Peanut Bark

Ingredients

1 Bag of Dark Chocolate Coating Wafers (16 oz.)

1 1/4 Cups Peanut Butter

1 1/2 Cups Dry Roasted Peanuts

2 Cups Crisped Rice Cereal

2 Cups Mini Marshmallows

Directions

1. Prepare Baking Sheet: Line a 9 x 13 baking sheet with wax paper then spray with cooking oil. Wax paper should extend beyond the ends of the sheet.

2. Melt Chocolate: There are two basic ways you should melt chocolate either

a) with a double boiler- add a small amount of water to the bottom pot filling only a few centimeters up the side. Simmer on low heat until vapors rise from water. Place smaller pot on top and add chocolate. As chocolate softens stir. DO NOT OVERHEAT or chocolate will seize and burn. Once completely liquid remove from heat immediately, or

b) use microwave. In microwaveable medium bowl heat chocolate for 1 minute on 50% power. Stir. Chocolate will NOT be completely liquid yet. Continue process of melting on 50% power in 30-second intervals then stirring. Every microwave different. You must heat up slowly, that's why you carefully heat on half power or you could burn the chocolate.

3. Add Other Ingredients: Stir in other ingredients one at a time to obtain a consistent blend in the following order peanut butter, peanuts, crisped rice cereal and marshmallows.

4. Pour into Baking Sheet: Pour mixture onto one end of the baking sheet then tilt it so the mixture flows down to the other end. Tilt sheet back and forth until chocolate mixture is even. Set aside and let chocolate set (cool). Once hardened drop sheet on top of counter to crack the chocolate.

5. Cut into Pieces (optional): If needed lift chocolate by ends of wax paper and place onto counter top. gently crack with butter knife. Should end up with many jagged-edged pieces of candy.

Now package them in one of the forementioned ways.

Biophysics professors at Western University, in London, Canada, have developed a CT (Computed Tomography) scanner small enough to sit on a desk. Jerry Battista, Chair of the Department of Medical Biophysics at the Schulich School of Medicine & Dentistry and Kevin Jordan of the London Regional Cancer Program at London Health Sciences Centre invented the DeskCAT Multi-slice CT Scanner as a novel and interactive way to teach CT imaging techniques to a wide range of students. DeskCAT is now being manufactured, and distributed to other universities by Modus Medical Devices in London, Ontario.

Clinical CT or CAT (Computed Axial tomography) scanners are large enough to handle a patient and occupy a large room. The donut-shaped scanner rotates a narrow fan beam of x-rays around the region of the body to be visualized. The x-rays are then detected and analyzed by a computer to create detailed images of the body part in thin slices, which can be stacked together to form a three-dimensional (3D) image.

The DeskCAT scanner was invented out of educational necessity. "Teaching the basics of a CT scanner is very complicated. Usually you end up filling a board with equations, and students get lost," explains Battista. "It's hard to get access to a clinical scanner for a more practical explanation because of the heavy clinical workload. So basically, we miniaturized a CT scanner to bring it into the classroom."

Rather than using x-rays, the DeskCAT educational scanner uses visible light rays to form multiple views of a transparent specimen. The mathematical method of reconstructing the 3D picture of the specimen's interior from many views through the object, is identical to that used in the full-scale clinical x-ray system.

"The advantage of using light instead of x-rays for teaching is that the scanner can be brought into the classroom or laboratory without the hazards of x-ray exposure," adds Battista. "Another advantage is that students can 'see the light' passing through the specimen whereas x-rays are invisible to the human eye. This provides unique insight!"

One Sugary Drink a Day Can Raise Heart Disease Risk - Healthy Fit

For 22 years, researchers from Harvard School of Public Health tracked nearly 43,000 male participants, ages 40-75 in a health? They also collected blood samples from more than 18,000 men who were demographically similar to the other participants.

Just 12 ounces of regular soda, fruit drinks and other sugar-sweetened beverages daily was associated with a higher risk of heart disease, even after controlling other cardiovascular risk factors, such as smoking, physical inactivity, alcohol use and a family history of heart disease.

The news isn?t much better for women either. Another study from 2009 found that women who drank one or less than two sugary drinks per day had a 23 percent increased risk of a heart attack.

?This study adds to the growing evidence that sugary beverages are detrimental to cardiovascular health,? said Frank B. Hu, M.D., Ph.D., study lead author and professor of nutrition and epidemiology in the Harvard School of Public Health, in a press release.? ?Certainly, it provides strong justification for reducing sugary beverage consumption among patients, and more importantly, in the general population.?

To stay on the safe side, The American Heart Association recommends no more than half of discretionary calories (calories left in your ?energy allowance? after consuming the recommended types and amounts of foods to meet all daily nutrient requirements) come from added sugars. For the average American man, that means no more than 150 calories a day, and for the average woman ??100 calories a day.

When oil started gushing into the Gulf of Mexico in late April 2010, friends asked George Haller whether he was tracking its movement. That's because the McGill engineering professor has been working for years on ways to better understand patterns in the seemingly chaotic motion of oceans and air. Meanwhile, colleagues of Josefina Olascoaga in Miami were asking the geophysicist a similar question. Fortunately, she was.

For those involved in managing the fallout from environmental disasters like the Deepwater Horizon oil spill, it is essential to have tools that predict how the oil will move, so that they make the best possible use of resources to control the spill. Thanks to work done by Haller and Olascoaga, such tools now appear to be within reach. Olascoaga's computational techniques and Haller's theory for predicting the movement of oil in water are equally applicable to the spread of ash in the air, following a volcanic explosion.

"In complex systems such as oceans and the atmosphere, there are a lot of features that we can't understand offhand," Haller explains. "People used to attribute these to randomness or chaos. But it turns out, when you look at data sets, you can find hidden patterns in the way that the air and water move." Over the past decade, the team has developed mathematical methods to describe these hidden structures that are now broadly called Lagrangian Coherent Structures (LCSs), after the French mathematician Joseph-Louis Lagrange.

"Everyone knows about the Gulf Stream, and about the winds that blow from the West to the East in Canada," says Haller, "but within these larger movements of air or water, there are intriguing local patterns that guide individual particle motion." Olascoaga adds, "Though invisible, if you can imagine standing in a lake or ocean with one foot in warm water and the other in the colder water right beside it, then you have experienced an LCS running somewhere between your feet."

"Ocean flow is like a busy city with a network of roads," Haller says, "except that roads in the ocean are invisible, in motion, and transient." The method Haller and Olascoaga have developed allows them to detect the cores of LCSs. In the complex network of ocean flows, these are the equivalent of "traffic intersections" and they are crucial to understanding how the oil in a spill will move. These intersections unite incoming flow from opposite directions and eject the resulting mass of water. When such an LCS core emerges and builds momentum inside the spill, we know that oil is bound to seep out within the next four to six days. This means that the researchers are now able to forecast dramatic changes in pollution patterns that have previously been considered unpredictable.

So, although Haller wasn't tracking the spread of oil during the Deepwater Horizon disaster, he and Olascoaga were able to join forces to develop a method that does not simply track: it actually forecasts major changes in the way that oil spills will move. The two researchers are confident that this new mathematical method will help those engaged in trying to control pollution make well-informed decisions about what to do.
